Elder Care Case Management, Advocacy and Mediation Services -- What would you look for with this type of service? What would you be willing to pay out of pocket?

    I would agree with Karen if the cleint can afford our services. Please be aware that Geriatric Care Managers do not have a standard educational requirement so do your homework when assessing qualifications.

    Our rates in the Pittsburgh, PA area are around $80 - $120 hour, depending on qualifications. If they are low income look towards your local area on aging to assist.

    Hello Karen,

    I am a member of the National Association of Professional Geriatric Care Managers. The web site is CareManager.org . You want to ask if the Care Manager is certified. The cost depends on location. In Northern VA the cost ranges from 100-175 per hour. Hope this helps.

    As an HR practitioner working in the community health care field, I can attest to the large need for these services. In Canada there is an abundance of senior services to assist with these very needs. However, despite a more inclusive health care system, a senior must be ill and unable to care for themselves independently to receive services of social workers and other community resources that will assist with decisions, often free of charge.

    There is of course private pay options, but much harder to tap into professional resources. Always better to use an agency with bonded staff with criminal record checks to assist with moving personal possessions, or any other job in the house. And get references!

    You might expect to pay about $22.00 / hour for someone to assist you, and up to $100.00 an hour for Social Workers.

    Best advice, subscribe to those seniors magazines and associations and network!

    One of the biggest problems that consumers have is knowing what type of service is the most appropriate, for the "current" situation. Geriatric care managers provide a comprehensive assessment, develop a care plan, identify resources and can monitor a situation (including moving/cleaning up/estate sale services). They are advocates for a person (or the person's "stuff").

    An elder mediator is an impartial third party who helps people who may have different ideas or thoughts about planning/doing for an elder (or an elder's "stuff"). A mediator will not provide an independent assessment of the elder's condition nor monitor the progress of a care plan, but rather will develop and facilitate a process for the parties to discuss solutions to any conflicts that exist. By involving the parties in such a manner, there is a higher likelihood that the parties will be able to work well together in the future.

    Geriatric care managers will certainly work to resolve conflicts and elder mediators will recommend services. But, it is important to distinguish between the advocacy role and the impartial role.

    Debbie's right on target. Care Managers can be very helpful as advocates/system navigators. Mediators have a different role and different tasks that some times CMS might assist with where conflicts are not too complicated or tense. However, Elder Mediator's are called upon to deal with things that benefit from some knowledge of the law, as well as other issues perhaps requiring somewhat different training (or a ton of experience), e.g. : Estate Matters – Care Disputes – Provider-Patient Disputes – Chronic Illness – Conservatorship – Guardianship – Financial Decisions – End-of-Life Choices – Insurance – Power of Attorney – Serious Sibling & Parent-Child Conflict – Family Reconciliation.     Absolutely Karen. I would be very interested in participating in any initiatives to make Seniors lifes easier and promote access to services. Very often its lack of knowledge surrounding the availability of resoures/services that is disservice.
